1) Proof of appointment printed from the system (COPY);
2) Money order or cashier’s check (inside an envelope);
3) Application Form, Forms 1, 2, 3, 4
(all 5 forms must be signed in front of a notary public and notary public’s signature must have an apostille (ORIGINAL);
4) Two copies of current passport (U.S. and/or from another country) (COPY);
5) U.S. Certificate of Naturalization, if you became a U.S. citizen by naturalization or if you underwent naturalization in a different country (ORIGINAL); the original one will be sent back to the applicant.
6) Two copies of current Green Card, if not a U.S. citizen;
7) Two copies of current driver license or state ID;
8) ORIGINAL recent utility bill as PROOF OF RESIDENCE
(ONLY electricity, water, gas, or landline bill) (page with applicant’s name and address)
OR alternatively current proof of enrollment in a university/school, lease contract, and voter registration card;
9) Applicant’s birth certificate (with apostille and translation) (ORIGINAL)
10) Last ancestor born in Italy (ORIGINAL):
a) Italian birth certificate;
b) Marriage certificate (with apostille and translation if it is not an Italian document);
c) Death certificate (with apostille and translation);
d) All documentation regarding naturalization or lack of naturalization (see Checklist 2);
11) All other ancestors until applicant (ORIGINAL):
a) Birth certificate (with apostille and translation);
b) Marriage certificate (with apostille and translation);
c) Death certificate (with apostille and translation).
